In a limited tournament, is it mandatory for heroes to spawn from an infantry unit even though their nationality has no infantry in the set? (Italian hero in set V, USSR and Japanese hero in set IV, eg)

Yes, unless the tournament organizer stipulates a special rule to allow it.
