Well, Italy's my favorite country to play, so I can give many different ideas :D.
Personally, I like an alliance with Turkey to take out Austria, with an aggressive opening, and then turn on Turkey with Russia's help. An aggressive opening toward Turkey (typical Lepanto, say), tends to get Italy nowhere, as Austria can grow large fast, and then doesn't need Italy. And Italy-Russia alliance can be very good, but it tends to expand Russia very quickly, who, again, doesn't need Italy, except for more centers.
Aggressive is the best opening for Italy, in my opinion, but if you're going to open defensively you should keep Venice occupied with an army, and convoy your other army to Tunis, via the Ionian Sea. This will keep all your future possibilities open, including an attack on France (which, in my experience, only rarely works, but could be worth a shot).

Everygame I've played Ive lost as Italy. Except for one, where me and a group of good friends teamed up. Me(Italy), my friend(Russia), ditto(England). I forget how we won though....

As with so many other tactical questions in Diplomacy, the answer is "it depends". LOL
I have seen people be successful as Italy both uising a cautius approach, and by moving hard against an opponent immediately.
