When a character falls through the Collapsed Room, and lands in the basement, before any other part of the basement has been explored, where is the tile placed? Is it put next to the Basement Landing tile, or what?
Also, when one ascends the stairs from the Entry Hall to the second floor, does he automatically enters the second floor landing, or does he place a random tile?

1)You would draw a tile and put it next to the "basement landing" tile.
2)Yes...he/she goes to the "upper landing" tile...and continues any movement from there.
[Hope that helps]- ;)

What if all the basement cards have already been played when the collapsed room is drawn?
Edit: I have another question, as well. In rooms such as the junk room or the pentagram room where you have to roll when exiting to avoid taking damage to a trait, if you fail your roll can you choose to stay in the room and try again on your next turn?

And here's another question: what if the Secret Passageway comes up, when no rooms in the basement (other than the landing) have been explored? We assumed you drew one and placed it next to the Landing, but that's kinda silly, as you'll have a door, and a secret passageway...

I don't have the rules with me, so I can't answer your main question right now. But the answer to the second question, I think, is that you're not stopped by the roll: in other words, when you exit the room you roll for damage; if you fail the roll, you're in an adjoining room, injured. You can't stay in the damage room and re-roll next turn because you're no longer in the damage room. Make sense, or did I do a lousy job answering? Basically, staying and re-rolling would mean stopping in mid-step at the threshold of the room, spinning around, and trying to face the peril all over again. (Which wouldn't be so bad if the peril were a group of insane nuns wanting to be punished for lighting the Grail beacon again. Which it presumably isn't.)
