Can a force trying to break the siege of a friendly town be joined by forces sortie-ing from the besieged town?

The rules would suggest (though not indubitably) no, but the reason I ask this is that otherwise only one force is permitted to attack (by the "no force can be forced to defend twice in a turn" rule). This seems neither fair nor realistic, and allowing the two forces to "join forces" for the attack would fix this.

Well, those are the rule though. You can make a new home rule for that situation, but fairness and Kingmaker are not exactly on the best of terms.

Immediately after a successful siege, may nobles who were in the besieging stack but not actually taking part in it enter the town?

I thought you had to make a decision with your stack, either to beseige or not beseige. A common tactic with the group I used to play with was to figure out the fewest number of nobles necessary to come up with a successful siege, and then split a stack so as to minimize the chance of a noble getting clubbed. This can be risky though, movement precedes combat so you're stuck with a divided force, more tenable during the early game or when your opponents are far away.
So no, I don't think you can join them back up until next turn.
