Don't think of it as "disrupted + disrupted." Think of it as "1 hit plus 1 hit."

The first hit against a unit causes a face-down disrupted marker to be placed on the unit.

If a vehicle already has a face-down disrupted marker on it and it takes another hit, then it receives a face-down damaged marker. If an infantry unit that has a face-down disrupted marker on it takes another hit, then it receives a face-down destroyed marker.

A vehicle unit that takes a third hit in a single phase receives a destroyed marker. (It's also destroyed if it takes hits = def x2 from a single attack.)

In other words, each time a unit gets hit in a single phase, it gets a marker one step worse than whatever is already on it.
